This tubby fellow with Patton Oswalt lips is found in a French illustrated manuscript dated 1454: Jean Mansel, "La Fleur des histoires ou les hystores rommaines abregies." This seems to be a book about the history of Roman princes, and I've no idea whether this guy's presence in the margin of folio 257r has any relation to the subject.
